I am the legal guardian of (singer) and do/do not give consent to the Edmonton Young Voices Choral Society to use the name/image/video of (singer). Edmonton Young Voices and its agents have permission to utilize my child's photographic/video image in Concert programs, in its public relations and communication materials for a period of five years from the date this consent is signed. I realize that I may withdraw my consent in writing at any time by contacting the organization.

I understand that the photograph(s)/video(s) may be used in a publication, print advertisement, direct-mail piece, electronic media (e.g. various picture & video digital files on the internet including our website, social media, and other marketing channels, etc.) or other communication, but children will never be identified by name.

In giving my consent, I hereby release and hold harmless the Edmonton Young Voices and their agents from any and all responsibility or liability for damage of any kind suffered in any manner whatsoever. I hereby relinquish any and all personal or proprietary rights I may have in connection with such use. I understand that I will receive no compensation should any photograph of my child be used.
