Acknowledgements

I understand that my registration is not complete until payment has been received and confirmed by the choir manager.

I acknowledge the $50 Lost Music Fee and $50 Lost Uniform Fee (Children's Choir only) will be assessed for unreturned music/uniforms.

I consent for EYV to use images and/or video of my children (individually or in a group setting) for promotional purposes.(see Media Release Form)

I understand that the family of each singer will be required to fulfill 2 volunteer positions. A $100 fee may be assessed for failure to fulfill your volunteer duty.
