Registration cannot be guaranteed until the $50 non-refundable deposit has been received. You will have the option of credit/debit card payment or cheque payment following completion of the registration form.

* Please note: tuition will be billed in a separate invoice after registering.

This may include information about rehearsals, concerts, choir functions, etc., as well as invitations to past choir members to register for the next session. (You can remove yourself from the mailing list at any time)

Acknowledgements

I understand that my registration is not complete until payment has been received and confirmed by the choir manager.

I acknowledge the $50 Lost Music Fee and $50 Lost Uniform Fee (Children's Choir only) will be assessed for unreturned music/uniforms.

I consent for EYV to use images and/or video of my children (individually or in a group setting) for promotional purposes.(see Media Release Form)

I understand that the family of each singer will be required to fulfill 2 volunteer positions. A $100 fee may be assessed for failure to fulfill your volunteer duty.
